- One Comprehensive Initial Exam
- One Additional Periodic Exam
- One Full Mouth Set of X-Rays

Adult Membership Plan B -$ 279
- One Comprehensive Initial Exam
- One Additional Periodic Exam
- One Full Mouth Set of X-Rays
- One FMD (Full mouth debridement) per year
Child Membership Plan C (13 & under)-$199
- One Comprehensive Initial Exam
- One Additional Periodic Exam
- One Full Mouth Set of X-Rays
- Two Regular Cleanings per year
- Two fluoride treatment per year
There are a few terms and conditions you should be aware of:
- All services must be scheduled and completed before the plan year ends.
- You will receive 20% off any additional exams and x-rays needed for diagnosis of additional treatments.
- Additional family members can be added at any time. The plan end date will not change.
- Your Membership Plan may not be combined with other offers.
- Your plan is not transferrable.
- There are no refunds, no exceptions.
- Your Membership Plan cannot be combined with dental insurance.
